HOSPICE PHYSICIAN
Collaborates with the patient’s primary physician and are available for pain and symptom management consultations.
NURSES
Our nurse offers caring support that prioritizes the comfort and dignity of patients and their families. Specializing in evaluating and treating patient’s pain and symptoms. In addition to hands on care, they work closely with the family members in educating and guiding them for treatment and care practices.
PERSONAL AIDES
Our hospice aides are certified nursing assistants who assist patients with personal care related tasks, such as bathing, oral care, and dressing. They help take the load off of family caregivers by performing housekeeping duties.
SOCIAL WORKER
Assists in providing emotional care to patient and family members. helps address financial issues. Helps obtaining any governmental support, if qualified. Assists with end-of-life preparations such as funeral and mortuary arrangements.
CHAPLAIN
Our chaplains offer emotional, spiritual, religious and existential support to all members of a patient’s circle of care. It is their job to find out what might be needed to put a patient and family at ease, and to help bring that about. They also provide expert bereavement counseling to our families.
Our team.
Our interdisciplinary teams (IDT) are coordinated by a physician and registered nurses with the sole purpose of addressing the individual needs of each patient and his/her family. The IDT develops a plan of care (POC) specific to the unique issues affecting each patient. Providing compassionate care is very important and our team aims to improve the quality of life for those with terminal illnesses by meeting their physical, emotional, and spiritual needs. This helps patients spend their last days in comfort and dignity, supported by a team of trained healthcare professionals. Family members also receive support to help them cope with their loved one's condition. Ultimately, our team focuses on personal choice and individualized planning, making sure that each patient's final days respect their values and wishes.
